Welcome to SiteSmith Forms

SiteSmith Forms is where you build contact forms, quote requests, sign-ups, surveys and RSVPs, share them with a link or put them on your website, and read every response in one table.

The Forms home page with stats, the This month usage card and three published forms

What you can do with Forms

  • Build a form from a template or from scratch, with 17 kinds of fields.
  • Share it with a link, embed it on any website, or show it on a WordPress site or a SiteSmith site.
  • Get an email for every response, and send responses to other systems with a webhook.
  • Read and sort responses: star them, mark spam, search and export to CSV.

What's free and what's paid

Forms has its own plans: Free, Pro and Business. Every account starts on Free, which includes 3 forms and 100 responses a month, plus one API key. Forms on the Free plan show a small Made with SiteSmith Forms line at the bottom.

Pro and Business raise those limits and remove the SiteSmith line. Email alerts, webhooks and CSV export are on every plan. The Plan page in Forms shows your current limits and the prices.
